Prescribed application forms can be obtained from the Office of the District Welfare Officer for Differently-Abled, Pulikeshi Road, Tilaknagar or downloaded from the Department website — www.dwdsc.kar.nic.in and the filled-in forms should be submitted before Oct. 15.
